Step One

Connect

Complete our self-referral form and send to us or ask your GP to send a referral directly.

Our team will contact you to make an appointment and supply you with a form to fill in prior to your first visit.

Step Two

Nurse intake interview

Appointment with a specialist nurse who will identify your key concerns and priorities, provide education and resources, and be a patient support coordinator.

No out-of-pocket cost

Step Three

GP appointment

Specialised GP will see you to create a tailored management plan

Necessary tests, referrals and medications will be arranged

Step Four

Multidisciplinary classes

Grant funded group classes including pelvic physiotherapy and education, pain physiotherapist education, nutrition for pelvic pain, vagal nerve breathing and stretch class and yoga for pelvic pain, are available.

Step Five

6 week nurse check-in.

If needed, our nurse will touch base at the 6 week mark to see how you are doing

Step Six

GP results follow-up appointment.

Step Seven

3, 6, 9 month review

You will catch up with your GP and nurse at 3 months to review, assess, and consider next steps.

Step Eight

12 months post entry

Review, fine tune, and ensure comprehensive handover to usual GP.
