45 minute comprehensive assessment with a specialist GP.

Subsequent consultation.
30 minute follow up with specialist GP.

Quarterly planning and lifestyle review
Planning, coaching and lifestyle review.
We encourage you to come in for a quarterly planning, coaching and review appointment with our specialist nurse and GP.

Catchup with our specialist nurse to review symptoms and optimize wellbeing.

Multidisciplinary team review
Multidisciplinary case conference.
If required, we will (with your consent), discuss your case at our monthly multidisciplinary team meeting, to ensure everyone is on the same page, and to gain the benefit of multiple perspectives and areas of expertise.

Specialist nurse will touch base after the multidisciplinary meeting to discuss outcomes and agree on a plan moving forward.
